After installing the game, run the English patch (it should have installation instructions in the readme or something, usually they have simple installers) and that's it.

When you have unpacked disc images, just mount them in virtual drive and run the setup (important: you have to install in Japanese locale, unless the VN was released officially). virtual CD/DVD drive (programs like Alcohol or Daemon Tools) 002 etc., to merge them you may need HJ-Split (quite useful little program).

depending on your source, sometimes archives are split into parts that end with. an unpacking program ( 7zip will be the best, Winrar can occasionally mess up the non-unicode symbols). When it comes to installation, you should download the image of the game's CD/DVD (or said image compressed into zip/rar archives) and the English patch (the best idea is getting them directly from translator's site, that way you'll have the latest version). And I don't mean just the H-scenes (those are quite horrible btw., but they have absolutely EPIC BGM XD).

The extended version don't only adds new routes - as far as I know, it also adds/changes some stuff in the routes from the basic game.

Ecstacy seem to be stalled at the moment unfortunately. You can also follow current translation project there.
